Physiotherapy post COVID-19 – How we can help?

COVID-19 and Physiotherapy

Patients who have been infected with the coronavirus and admitted to the hospital for this reason should receive physiotherapy as soon as their condition allows. Seriously ill patients who are on ventilators and are confined to a hospital bed for long periods experience problems with breathing and other physical issues, due to their prolonged inactivity. Physiotherapists can be of great value to patients and their care in this regard. Physiotherapist assist with recovery in the ICU, but what happens when a patient is sent home?…

Once a patient is discharged from hospital it is vitally important to begin the rehabilitation process!

Recovery after intensive care

At first, patients will have very little energy. People on bed rest for the entire day lose about 5% of their total muscle mass and general condition each day. This will result in weakness and fatigue. Patients may not only suffer with muscular deconditioning, but may also experience respiratory complications post discharge. This condition is known as “post-intensive care syndrome” (PICS). A rehabilitation program designed and implemented by a physiotherapist compromising of muscular strengthening, respiratory exercises and functional independence training would be holistically beneficial to the patient going home!

Physiotherapy care will focus on the patient being given exercises aimed at improving lung function by strengthening the respiratory muscles, coughing up phlegm more effectively and breathing more deeply, thereby improving the ventilation of the lungs. They are also given motion exercises.

How can our Home physiotherapist help you

A big focus of physiotherapy during the pandemic will be to get patients moving as soon as possible. But this can only happen when the patient is well enough.

This can start with simply getting patients to move their arms, legs and body in the bed. Physiotherapists will closely manage vital signs such as oxygen levels, respiratory rate and blood pressure to ensure movements are safely tolerated.

Rehabilitation will also vary depending on the patient’s condition. Tiredness or fatigue is reported in about 40% of patients, so physiotherapists will pace activity and potentially see patients more often for shorter treatment sessions. Rehabilitation will be carefully planned and progress gradually to the patient sitting on the edge of their bed, standing, marching on spot, and walking, depending on how well each is tolerated.

Physical strengthening programs will be monitored and progressed until the patient can return to full activities of daily living!

You can also avail our Home PCR test when you are in Dubai

Home Care Services in Dubai – What You Need to Know

Home care - Dubai
Home care – Dubai

Expert home care and healthcare services in Dubai is difficult to come by. The thought about returning home after a stay in any hospital or clinic is most endearing to any patient. Whatever the nature of the illness or the health condition of the patient, it is both more cost-effective and reassuring for the family to get the patient back home.

Everybody is comfortable of the known and emotionally connected environment that a home provides. But it is better said or wished than done. It’s a daunting task to care for the sick and dependent patient at one’s home, while also being attentive to the patient’s recovery and providing her with ample support to pursue normal activities. So, in comes health care agencies that provide capsule packages for home care depending on the type or severity of the condition.

In a city like Dubai this is especially true for expat families who are mostly two members and with or without children. So, it’s not possible for one of the partners to take a long break from office and even if one is able take that, taking care of children at home as well as caring for the sick one is not a possibility. And if an elderly person needs such medical attention and care at home, it is all the more imperative to get professional home health care services to provide care and nurturing support to elders who are often more sensitive than not. Plus, there are certain conditions like Parkinson’s disease, diabetes or Alzheimer’s disease require specialized care from trained medical nurses in home.

The most common features or services  a top-quality home care health agency in Dubai must include:

Case Analysis and Patient Coaching:

The foremost thing is to take in stock of the situation and analyze the patient’s case and design a tailor-made template for her. An advanced health care agency must also ask relevant questions to the patient party to understand the basic environment at their home and derive a fair idea about their lifestyle before designing a package for them. It also helps to coach the patient beforehand to break the ice and make him mentally sign up for the upcoming process and set health goals together in order to keep her motivated.

Trained Nursing Care:

All-round in-home nursing services must include maintaining daily log of the patient’s vital parameters, signs and symptoms as well as proper management of such symptoms and preventing further complications.

Elderly Care:

This is a major area which requires not only medical proficiency but a positive body language and attitude and a sensitive behavior as elders are susceptible to loss of self-esteem in such situations. A well-mannered home care nurse should be able to make a patient feel comfortable while assisting them with day-to-day activities and private rituals as well as self-grooming. Daily home elderly care services include assisting with daily living, dispensing of medication, diet monitoring, helping with movement and prevention of bed sore, as and when applicable.

Palliative Care:

Specialized home palliative care is an important part of home healthcare services as this form of alleviation and soothing of pain makes the journey of life more bearable for patients suffering from terminal illnesses. This also involves a psychological tuning as one needs to spread a message of positivity as far as possible while respecting the patient’s dignity and stay true to medical supervisions.

Cancer Patient Care:

Cancer patients require specialized care and medical attention as a slight change or unwarranted move might disturb their process of recovery and sense of equilibrium. Holistic healthcare for cancer patients at any stage also require an alert and active trained home care nurse.

Maternity and Infant Care:

Stay-at-home maternity care nurses are required by most families post childbirth for optimal postnatal care that includes assisting the new mother with lactation, breastfeeding and providing her ample rest such that she can recover fully. Monitoring the baby’s feeding and sleep cycles as well as helping parents adapt to the multiple needs and moods of babies is a process that home care maternity nurses attunes them with.

Alzheimer’s Disease Care:

Conditions like Alzheimer’s disease require round-the-clock monitoring of not just the patient’s day-to-day activities but also engage her with mental activities like word games and number games and thus help them not to sink into a psychological loop. Plus, such patients often require attention for going out.

In Nightingale Dubai, we have home care & health care nurses with strong credentials and with a proven track record of having handled delicate medical conditions. International Nurses Day 2021

To mark this year’s International Nurses Day on May 12th, we’ve asked three of Nightingale’s nurses to share a glimpse of their typical day and tell us what they love most about the job.

Malin Ghavami (Head Midwife)

My day frequently starts with a home visit to new mums, usually when their little one is less than four days old.  We discuss the delivery and their recovery, and I offer any help and encouragement they need about things like feeding techniques, monitoring the baby’s weight and progress. After each visit, I write up my notes and tell the family when I’ll be visiting them next.

nurse malin

Afterwards, I schedule baby massage sessions for infants who are a few weeks old and sometimes a postnatal massage for their mums. I also manage the nurses in the community, so I dedicate time to training, or we have a general catch-up meeting. Training topics range from perfecting swaddling techniques to discussing clients’ specific needs and how best to address them.

As the day winds down, my colleagues and I discuss the schedule and requirements for the next day, although that can change entirely when a little one needs special attention earlier than planned.

I chose to be a nurse because I wanted a profession that I could be proud of, where I can positively impact people’s lives, be it the mummies or their sweet little babies. There’s no better feeling.

Julia Youll (Public Health Practitioner)

I wanted to be a nurse ever since I was six years old and was given a nurse’s uniform for Christmas, complete with a medical bag.  My grandfather was a GP and I was fascinated by the surgery and all the paraphernalia (in those days kept at his home). As soon as I turned 17, I started applying to teaching hospitals in London.

We were trained at the hospital and had a lot of practical time on the wards, interspersed by study blocks.

We certainly weren’t supernanny as students in those days, and we wore stiff starched detachable collars, cotton frilly caps and proper aprons!

I usually start my morning checking in with the families that I’m supporting with sleep problems, and it’s always great to hear that a good night has been achieved. Everyone has a new lease of life when they’re feeling rested.

My home visits (or calls) usually take place in the afternoons. It’s really productive to see babies and their parents in their home environment. After an hour or so’s discussion, I go away and create a plan for families to work with. This is tailor-made, specific to each one’s circumstances and dynamics. Follow-up may entail tweaking the plan, or looking at different ideas, but in general, a family without sleep deprivation is a happier one. Even a small change can make a huge difference.

Hermina (Nurse)

Working as a nurse at Nightingale gives me the opportunity to support families during an incredibly important yet challenging time ,and it’s wonderful to see them experience so many beautiful firsts together.

Baby Afra, born earlier this year, was premature and underweight. She was so small and delicate. The first time I held her, I could feel her vulnerability and how much love and care she needed. I fed her every three hours with precise measurements of milk and the correct feeding technique, checking her weight every ten days. She started gaining weight and every day she amazed us with her smile. Now, when I look at her playing and giggling, I can’t explain the happiness I feel having being part of her life. Moments like these are why I decided to become a nurse.

Why Should I Take Flu Vaccine

It’s again the time of the year for that jab (We know you don’t like it though). Taking a flu vaccine in Dubai has lots of merits.

This year amidst the corona virus/Covid-19 pandemic, it is more important than ever to take your flu vaccine. Dubai Health Authority (DHA) has made it easy and affordable for everyone. By taking the vaccine, you are not just protecting yourself, but your family, friends, colleagues and the community (especially for your children).

Few facts about flu and the vaccine:

  1. The flu – short for influenza – is a viral infection that affects your respiratory system (Ring any bells? We will come to that later). Seasonal influenza outbreaks usually happen in winter. Starts as early as October and continue till May. Flu symptoms vary from asymptomatic or mild through to severe symptoms and complications that can lead to death. Laboratory testing is required to confirm a diagnosis.
  2. There are around 5 million cases of flu reported every year which result in hundreds of thousands of deaths.
  3. Talking about the respiratory system, Flu and COVID-19 are both respiratory illnesses, but are caused by different types of viruses. It may be difficult to tell the difference between them as symptoms of flu and COVID-19 are similar. A flu vaccine does not protect against COVID-19.
  4. There are mainly four types of influenza viruses, types A, B, C and D. Influenza A and B viruses are the main cause of seasonal epidemics. And there are more than 100 subtypes, and the ones in circulation change from season to season. Now you know why we get the flu every year and the importance of getting the flu vaccine.
  5. The flu vaccine contains a deactivated form of the virus (as in almost all the vaccines – though chickenpox or measles vaccine contains a weakened live version of the virus), which teaches your body to recognize a real flu virus and respond. Way back in 1933 the very first generation of live-attenuated vaccines was developed; and in 1942, a bivalent vaccine was produced after the discovery of influenza B.
  6. Everyone starting from the age of 6 months is advised to take the vaccine.
  7. Every year scientists meet and analyze the current year’s data (142 national influenza centers in 113 different countries!) and prepare the composition for the yearly vaccine. Fun fact is that for northern hemisphere the composition is decided in February and southern hemisphere in September.
  8. Will there be flu along with COVID-19 in the fall and winter? – Yes and that’s why we are asking you be on the safer side. For you and for the community. If you contract Covid -19 and Influenza at the same time you may become quite unwell, hence best to get vaccinated and do PCR tests for COVID.
  9. The actual cost of the influenza vaccine in Dubai is Dh 21.60. Total cost varies with the facility’s additional charges. Read Gulf news report (https://gulfnews.com/uae/health/flu-season-in-uae-why-influenza-vaccine-prices-differ-across-clinics-in-dubai-1.73847763 )

Age Related Risk of Covid-19 (corona virus) infection

*The data listed here might change from time to time and is sourced from different places.

Probably the biggest influencing factor in terms of disease outcomes is how old you are. The older you are, the worse your chances of surviving it.

In other words, if you are aged, for example, between 65-74, you have 198.7 chances in 100,000 of being hospitalized, for the virus, during the period up to 6 June 2020. Let’s make that easier to grasp – 198.7 in 100,000 is the same as 0.2% – one chance in 500. That’s not quite so scary a number, is it.
This is not a meaningful number, unfortunately, because we don’t know what percentage these numbers are of all people getting the disease in each age group. Any hospitalization first requires you to get the disease and then to become sufficiently seriously unwell and require being admitted to a hospital. To tell us the overall number of people being hospitalized is a great example of a statistic that may be perfectly correct, but which is also unhelpful to the point of being meaningless!
This chart below gives a very vivid depiction of the bottom-line impact of age on death rates.

Physiotherapy post COVID-19
Physiotherapy post COVID-19

Compared to people aged 18 – 29, if you are 65 – 74, you’ve a five times greater rate of being hospitalized and a 90 times higher risk of dying. And if you’re over 85, the number is astonishingly greater again. It can also be derived from this chart that the risk of dying, after being hospitalized, goes up with age as well. You see that 30 – 39 year-olds have twice the chance of being hospitalized, but four times the risk of dying, while 50 – 64 year-olds have four times the chance of being hospitalized, but not four times the risk of dying, but a massive 30 times more.
These numbers are helpful, but there’s still a vital pair of data points missing. What is the risk of hospitalization or dying for the benchmark 18 – 29 yr old? We can’t conveniently determine that from this chart. So yet again, while we know the risk is much higher or lower depending on your age, it is difficult to put an exact number on it, and of course, that’s what most of us instinctively would wish to have.
We need to point out something that is little appreciated. While it is true that very few children seem to contract severe or fatal cases of Covid, that does not mean they are not at risk. There are a couple of important considerations.
First, an infected child can still pass their infection on to other people. Second, there is increasing awareness of recovered children now showing signs of various lasting harm and damage to their arteries and veins. Another source studied the chance of patients dying in a number of countries. The word “patient” isn’t defined, but we assume it mean “someone who has been admitted to hospital” rather than referring to everyone known to have been infected.
There are surprising differences in death rates in different countries (huge differences – in some countries you have a ten times greater chance of dying than in other countries) so we hesitate to consider the numbers in the study as anything other than indicative and confirming the general concept that older people are much more at risk.

age related risks of covid
age related risks of covid

As you can see, there is a similar massive increase in mortality with age. The actual values are unfortunately quite different from the previous set of data (why?), but the concept of major increases in risk/mortality with advancing age is consistent. There’s one more thing to keep in mind. We don’t know for sure (not yet, anyway!), but we’ll guess that when you’re over 80, your chances of dying if hospitalized for any illness are very much greater than if you’re half that age. So are the higher numbers we’re seeing here “just because that is how it goes for everyone and every disease” or are they truly/significantly higher because the coronavirus is more dangerous?  That’s something that would be helpful to know, but not something any of the studies we’ve seen have chosen to consider.

Home care – Dubai

Home healthcare over the last decade has improved a lot. ROLES AND RESPONSIBILITIES OF IN-HOME CARE NURSES

Before choosing a nurse for you suffering loved one, the first thing that you need to ensure is to have a detailed discussion about the health condition of the patient with the home healthcare agency by producing all the investigation reports and doctor prescriptions to avail a home nurse specialising in care for a condition that your patient is suffering from.

On a general basis, in-home care nurses can be broadly divided into three categories.

I. ADL
II. IADL
III. Companionship

ADL ( Activities for Daily Living or Self Care)

If your patient requires something more than medical care like help, support and care related to daily activities such as bathing, mobility etc, then you should go for a nurse specialist in ADL care.

Basic ADL activities include:

  • Support with the feeding of the patient
  • Mobility of the patient that may include getting up from the chair or out of the bed. The nurse would assist the patient with the correct postures of movement etc.
  • Assistance with dressing and choosing the right fabric of the dress for better comfort.
  • Maintaining the highest standards of hygiene during this susceptible phase of the patient. Helping the patient to maintain toilet hygiene and avoid various infections such as a Urinary tract infection or maintain personal hygiene such as hair styling, brushing etc.
  • Assistance with daily activities such as bathing and shower.

IADL ( Instrumental Activities of Daily Living)

These mostly support activities instrumental to daily living and functioning.
Such activities include:

  • Maintenance of cleanliness in the room where the patient is living to create a dust-free environment with minimum germs inhabiting in the area. Maintaining strict discipline to ensure visitors family members frequenting the patient room follow certain do’s and don’t’s.
  • IADL home health care nurses prepare meals for the patient keeping in mind the diet of the patient at the same time including ingredients for a healthy diet of the patient and maintaining a routine and timing of meals for the patient. The home nurse may also go for shopping buying necessities and medicines for the patient with even responsibilities of managing the funds and expenses related to the patient. An IADL nurse may at the end of a week provide you with a summary of expenses that happened during the week.

Based on your requirement for the patient purely depending on medical monitoring, ADL or IADL choose your home caregiver after thorough consultation with your home care services agency.

Another category of home-care nurses provided by home care agencies is companionship based. Let’s know more about this category in details.

Companionship and positive mental health caregiver nurses

Companionship home care nurses ensure that they supply the positive spirit required by your patient to recover and get well. They also supply confidence to terminally ill patients such as the ones suffering from any kind of cancer to fight the disease with a positive spirit. Companionship also includes spending time with the patient, engage in positive conversations during hours when near and dear ones of the patient are out of the home due to work. Do keep in mind that these category of nurses are not necessarily only medical caregivers and their role goes beyond just providing assistance with activities to the patient. This category of nurses is also recommended for patient suffering from depression related to their disease or in requirement of any kind of psychological counselling.

We hope that this blog will now help you choose a home care nurse for your loved one more judiciously in a city like Dubai which provides home care services with registered ADL, IADL and companion nurses enrolled with them.

Benefits of Home Care for the Elderly in the UAE

Nightingale encourages the independence of our elderly clients while enabling them to remain active with their community and family.

elderly care services by a professional home nurse
elderly care services by a professional home nurse

Home health care for the aging and chronically ill in the UAE is quickly becoming the preferred solution for the growing elderly population. When provided the opportunity, most senior citizens prefer to live out their later years in the comfort and familiarity of their homes. With the help of a licensed medical professional, they can avoid having to move to an unfamiliar assisted living facility.

According to the World Economic Forum, “non-medical home care has been in formal existence for less than 20 years, but it is now helping millions of seniors around the world remain in their own homes, where they generally want to be, for as long as possible. Moreover, research indicates that non-medical care can very effectively complement medical care in today’s highly evolved senior care continuum.”

There are many benefits to obtaining home care for the elderly. From companionship to promoting aging with dignity, home care has multiple benefits. Here at Nightingale, we have compiled a list of the top reasons why you should use elderly home care to take care of your elderly relatives and loved ones.

Undivided Attention

When your loved one receives medical care in their home, the caregiver’s only responsibility is to provide compassionate, quality care for your loved one. This means your aging parent or loved one will have the complete, undivided attention of their caregiver. With a Nightingale caregiver, implementing care for the elderly is an individual, personalized affair. Your loved one receives one on one care at the necessary level to make them completely comfortable and safe.

Home Care is Cost Efficient and Personalized

A registered in-home nurse provides professional quality care and medical assistance. When compared to nursing homes and assisted living facility fees, they are an extremely affordable care option. On average, caring for your elderly loved ones in their home costs far less per hour than it would to admit them to a local elderly care home. Additionally, most reputable care providers in the UAE do not require the use of monthly or annual contracts. You simply enlist their services if and when you need them.

Home Care Provides Peace of Mind

When you bring a professional, preferred caregiver into your loved one’s life, you’re doing more than securing premium care for their health and wellbeing. You’re also guaranteeing the peace of mind that comes with knowing your elderly loved one is properly cared for. As our loved ones age, it becomes increasingly difficult to provide the range and level of health care they need and deserve. Taking advantage of a professional in-home nursing care not only bolsters their love and support with professional, quality care but also enables you to spend your time enjoying their company without the pressures of providing for their care.

Keeping Families Together

During difficult times, such as illness or old age, a powerful family bond can make all the difference in the world. Families provide a stable foundation for the emotional support of their elderly loved one. Knowing that your loved one is in an environment where they feel safe and secure is worth its weight in gold. Those who choose home care for the elderly over assisted living facilities and nursing homes experience the advantage of visiting their family members whenever they wish, without the restrictions of visiting hours. Additionally, home care providers are devoted to involving family members in the care process of their loved one.

Independent Living

Perhaps the most important aspect of home health care resides in the fact that home care promotes an environment of independence, comfort and confidence for the elderly. This allows them to recover faster and grow old with dignity. The home environment is the least restrictive place for older adults to remain engaged with their typical daily activities in the community and with family and friends around them. With a range of home care services, your loved ones have the control and the ability to lead a meaningful and independent life well into old age. Home care services can range from medication reminders and bathing/showering helping with daily chores, meal preparation, and grocery shopping. Additionally, Nightingale nurses are licensed by the Dubai Health Authority (DHA). DHA registered nurses are qualified to administer medical care and treatments and are trained in extensive first aid techniques.

At Nightingale, our DHA registered nurses are trained in first aid, safety procedures and emergency response. We encourage the independence of our elderly clients while enabling them to remain active with their community and family, and would love to do the same for your loved ones too!
